Advance Directive and Do not Resuscitate Orders Explained
An advance directive outlines what wishes your doctor must follow if you become unable to make your own medical decisions. When you`re admitted to the hospital usually the staff will ask you if you have an advance directive, or you can hand your doctor and hospital staff a copy yourself if they don’t ask.
The laws and forms on advance directives are usually different for every state, its important you get your state’s specific form that comforms to your state’s laws.
The good thing about an advance directive is that you don’t stress out your family and loved ones by putting the burden of making medical decisions for you. If your 18 years or over you can prepare an advance directive.
A do not resuscitate order is like an advance directive but very specific. A do not resuscitate order is made for you to request if you want cardiopulmonary resuscitation (CPR). Usually if you don’t fill this form out the whole hospital staff is trained in CPR and also trained to revive you using CPR if your heart stops or you stop breathing. Do not resuscitate orders are accepted in all states.

Yes, I think they would try to save your life. First of all, they have to be aware you have one on file. Second, they have to know it is valid. This cannot be done in the few minutes they have to try to save your life. Most people have this directive because they have been sick for quite a while and they know things will not improve much, if at all if they have a cardiac arrest or something similar. Suicide is completely different and viewed as an abnormality in thinking that can be cured with therapy, after which you could have a wonderful life.

You can write your own will out and have it be legal and binding, just have it stamped by a notary, and witnessed also. And no your witness does not have to be anybody in the will.

Yes, a living will and an advance directive are basically the same thing. Some states also call it a "Directive to Physicians." It's basically a form or statement that you make that shows your intent to refuse medical treatment and to release health care providers from all liability if you become both (1) terminally ill, and (2) unable to communicate such a refusal.
You don't have to be in the hospital with an end-stage illness to do a living will. You DO, however, have to sign the document in the presence of witnesses and/or a notary public (each state has different laws on this) who will attest that you signed the document of your own free will and that you were of sound mind when you signed it.
Attorneys can prepare these documents for you, but you don't have to have an attorney do it. Many hospitals and health care systems offer fill-in-the-blank forms that conform to state law that will allow you to do it without hiring an attorney. You can also get living wills off the Internet. Best way to find one is to type in the words "living will" and also the name of your state.
